How do I apply for service? What information is required?

Applicants applying for new service or transferring existing service should call (352) 793-3801. During the call, a Member Services representative will request that applicants provide full Social Security Number (SSN), date of birth (DOB), and driver license (DL) number (or other government-issued ID number, if applicable), as well as contact information (Full Authentication includes: Complete SSN, DOB and Driver License Number).

    Apply for service. Is this your online form? I called and the wait time was 45 minutes. Please advise. Thank you.
